As far as I’ve ever heard, they’re not the ones holding it up. Courtney Love has never given her approval for the DVD to be released with the Nirvana material (and it’d be pointless without) and so it never has, simple as that.

A friend of mine has the original tape and put it on dvd for me. This is an amazing lineup of preformers.

Dave Markey talks about the delays here posted by "jack static" on loobiecore:

What’s the status of a DVD release for 1991? I read that Courtney Love hasn’t given approval.
To be honest, I don’t really know the reason why. I know that’s definitely part of it, but there are the Ramones in there and other concerns as well. It’s just a matter of time before it all gets worked out. Of course, Courtney sold a portion of her publishing to someone outside of the band, so now [there is] someone else to deal with, so it just goes on and on and on. And, unfortunately, I’m not dealing with that; it’s Universal, who absorbed Interscope, who absorbed [the film’s initial label] Geffen. That’s the thing when you put something out in that world; you just have to deal with that world. It’s a real test of patience.

I have for years thought I am in the film (shot of the back of my head as Babes In Toyland played at Reading…). VERY long red (ok ginger hair. Ha ha ha. Maybe it’s just wishful thinking!!!
